To obtain an AGENCY, INDIVIDUAL, APPRENTICE or JOURNEYMEN application you need to request an application in writing (include your name and full mailing address) with a $5.00 money order to:

When making a request for an application, you must specify which licensing package you want,
AGENCY, INDIVIDUAL, APPRENTICE or JOURNEYMEN.

For License Renewals:
You must send in a completed renewal form (only available by mail from the LSBPIE Board Office), a Continuing Education Compliance Form, (if any agency level licensee) a current copy of your Occupational License, and the correct fees including any late fees incurred.

Additional Individual Applicants:
Applicant must send in the 3 page Individual application, Sponsor Form, copy of driver's license and fees.

The Change of Employer form must be accompanied by:
Personal Information/Application form, Sponsor Form, Notice Of Change/Re-Employment Form and fees.

The Change Agency Name and Address must be accompanied by:
Agency Application form, new Occupational License with new address and/or name and Personal Information/Application Form for each individual licensed under your agency, and fees.

Transfer License Level, No Longer Employed, and Re-Hired forms are self-explanatory and state on the form if additional forms are to be attached. Please read the forms carefully to determine if and which forms are to be attached along with any additional items.
